| I would like to see if my current status of eligibility has changed due to a newly diagnosed condition by the doctor's admission to me last week! I had seen Dr. Ceasar Ercole and he told me that I had Cancer in the Prostate Bone,and I would like to know if I'm eligible for more financial assistance, two visit's with counselor's scheduled,and then also plan's for surgeory also at the 420 Delaware location at the University College Campus! Located accross from the 516 Delaware location-Phillipk/Warrenstein building! Misspelled? This will be added on to the condition claimed by Dr. Winton Calvert also on the 6th Floor, at the same location, in the clinic on the sixth floor! If need a release form this is a submittance only for the purpose dealing with University of Minnesota Medical Physician's! 1st and 5th Amendment/Illegal Search-Seizure Motion To Suppress-If needed!
